2421.PACT对有自杀行为的军事人员的住院治疗:多站点随机对照试验

The broad objective of the proposed study is to empirically evaluate the efficacy of a cognitive behavioral intervention, titled, Post Admission Cognitive Therapy (PACT), for military personnel psychiatrically hospitalized due to a suicide-related event with either a recent or a lifetime suicide attempt. The primary outcomes will be incidence of repeat suicide attempt(s) and number of days until a repeat suicide attempt. Secondary outcomes include psychiatric symptoms, repeat number of psychiatric hospitalization(s), hope for one s future, and acceptability of treatment. A multi-site, single-blind, randomized controlled trial will be the research design. A total of 218 individuals will be recruited from the inpatient psychiatric and traumatic brain injury (TBI) units at the Walter Reed National Military Medical Center and Fort Belvoir Community Hospital. Participants will be randomized into one of two conditions: (1) PACT + Enhanced Usual Care (EUC) or (2) EUC. The PACT+EUC condition will consist of six 60-90 minute individual cognitive behavioral therapy sessions administered over preferably three days during the inpatient stay and up to four telephone booster sessions. The EUC condition will consist of usual psychiatric care patients receive during their hospitalization, the assessment services provided by MA and/or PhD level clinicians, and case management services provided for one year by Bachelor s level research personnel. Follow-up assessments will be conducted at 1, 3, 6, and 12-month post discharge by blind PhD level clinicians.

There were studies on uses of complementary and alternative medicine therapy to treat mental disorder like anxiety, depression, and adjustment disorder, whereas there was no effective method to treat such mental disorders. This study proposed a therapy from complementary and alternative medicine to treat mental disorder by through interactions of five senses between therapist and patient. In this method sounding a certain six voices play an important role in healing and recovery. By applying chaos theory, we investigated time series of six vocal sounds consisted of six syllables that have been used in the therapy. On the basis of the reconstructed phase space trajectory for the subject's voices, we found chaos features in the six vocal sounds with correlation dimension of D2=3.02±0.8. Further, we studied the effect of spoken six voices on human status. A measure from RQA, diagonal length Lmax, was computed for the records of start and end of voices during consecutively spoken the voices lasting lmin long. The mean value of Lmax increased from 6.4 to 7.9, giving nearly 20increase in the end of voice in relate to start. Wilcoxon test showed the significant changes in Lmax with p =0.019. Our results implied that the six vocal sounds induce a decrease of divergence in pulsation waves, leading to a formation of order out of chaos in human body. The proposed five sense therapy is a noninvasive intervention that can be used to reduce anxiety levels and improvement of mental health. First, we studied effects of speaking using scalp- EEG measurement. Chaos analysis of EEG showed a largely enhanced largest Lyapunov exponent (LLE) during the speaking. In addition, EEG power spectrum showed an increase over most frequencies. Second, we performed case studies on mental disorder using the therapy. Running power spectrum of EEG of patients indicated decreasing power at end of treatment, implying five senses therapy induced relaxed and lowered energy in central neural system. The results agreed with patient's reports that there were considerable decline in anxiety and improvements in mood.

Global treatment and payment protocols are gaining interest as an efficient and effective method for healthcare delivery. Although Osteopathic Manipulative Therapy (OMT) has been traditionally provided on a fee for service basis, a global treatment and payment program for OMT may be suitable for the management of chronic pain. This research tests a protocol for a global treatment and payment model for OMT in chronic back pain showing it to be well received by family medicine residents and patients with the potential to serve as a protocol for future evaluation of global treatment and payment models for OMT for a variety of conditions.

The hypothesis underlying this synergistic partnership award was that immunotherapy of epithelial ovarian carcinoma (EOC) with chimeric antigen receptor (CAR)-expressing T-cells would be potentiated if simultaneous targeting of tumor cells and tumor-associated macrophages (TAM) was achieved. To test this hypothesis, we initially engineered T-cells to express CARs with specificity for MUC1 (expressed by tumor cells) and CSF-1R (expressed both by tumor cells and TAM). In-vitro experiments demonstrated some efficacy of this therapeutic strategy. To test efficacy in-vivo, we used a previously established (EOC-like) tumor model based upon MDA-MB-435 cells, engineered to co-express MUC1 and CSF- 1R. However, no significant therapeutic activity was observed in this model. As per an agreed revised statement of work, a CAR termed T1E28z was constructed which targets T-cells against several ErbB receptor dimers that are upregulated in EOC. Liposomal clodronate was used to achieve depletion of TAM. We showed that T1E28z-transduced T-cells were effective in mediating the killing of both autologus patient-derived tumor cell cultures and EOC cell lines (IGROV-1 and SKOV-3). Next, xenograft EOC models were established using luciferase-expressing SKOV-3 tumor cells. Using bioluminescence imaging (BLI), we demonstrated that T1E28z+ T-cells mediated the regression of established SKOV-3 tumors in SCID Beige mice. Whilst highly efficient depletion of TAM has been achieved using liposomal clodronate, this did not influence anti-tumor activity in this model. However, potency of immunotherapy was enhanced by repeated administration of T1E28z-engineered T- cells. T-cell activity was imaged in this study by co-expression of renilla luciferase in these cells. This analysis revealed that T-cells undergo progressive decline in tumor-bearing mice, providing a strong rationale for repeated administration to achieve maximum efficacy.

2425.靶向Stat3蛋白和HIF-1α对前列腺癌的联合治疗的进展

There are two signaling pathways: HIF-1alpha and Stat3 are important targets for prostate cancer (PC) therapy. HIF-1alpha is the key gene that controls the amount of the transcription of hypoxia-inducible genes. Overexpression of HIF-1alpha not only strongly enhances the rate of tumor growth and metastatic potential, but also contributes to resistance to radiotherapy and chemotherapy, leading to treatment failure and increased patient mortality. Stat3 participates in oncogenesis through the upregulation of genes encoding apoptosis inhibitors, cell-cycle regulators, and inducers of angiogenesis in many human cancers, including prostate cancer. Also, Stat3 suppresses anti-tumor immune responses and mediates the cancer-promoting properties. Previously, we demonstrated that treatment of PC in preclinical models with a combination of a Stat3 inhibitor and a HIF-1alpha inhibitor greatly enhanced drug efficacy and dramatically increased apoptosis in human tumors compared with the use of either agent alone, showing that targeting both Stat3 and HIF-1alpha together can improve tumor response. In this research period, we have developed TEL03, a dual inhibitor from nature products, for cancer therapy. TEL03 targets both HIF-1alpha and Stat3, blocks the expression of their down-regulated oncogenes and significantly suppresses tumor growth in vivo. TEL03 also demonstrated a greater-than-expected in vivo potency, perhaps because of synergy arising from the ability of this agent to simultaneously target two critical oncogenic pathways.

The existing parallel type pressurized water supply pipeline of high-rise buildings has been improved to build energy-saving high-rise building water supply systems.Series-parallel combined constant pressure water supply pipeline is designed.Antipollution high water tank is reasonably installed above the middle area of the building to supply water for the middle area high water tank and supply pressurized water by water pumps for high-rise district.And PLC control system is designed to control the work efficiency of the water pumps according to the sensor feedback information.Verified by calculation and simulation,series-parallel combined constant pressure water supply pipeline is better than parallel water supply pipes in saving the energy which is about the middle-Hybrid phase consumption of the latter.PLC control is simulated and the results show that PLC can effectively control the water supply system to ensure stable water pressure so as to save energy.

Research of college classroom intelligent Lighting control system is based on the point of view of intelligence and energy saving.Considering thc phenomenon of lighting waste and poor system management,a design of high stability and low power consumption intelligent Light control system is proposed.Through the research of distribution method of light source and the application of CAN bus communications technologies,this paper mainly introduces a distributed light control system based on ARM.According to the demand of indoor illumination,the system can achieve real-time and accurate regulation of all classrooms' lights.Compared with the traditional lighting control system,besides the characteristic of energy saving,the system also is characterized by strong anti-interference and intelligent management etc.
