    In biology textbooks, we are taught to believe that biology is all about chemical reactions. However, recent discovery of electromagnetic sensing and antenna like communication of key biomaterials demand for the existence of a world of physical communication and energy exchange squarely parallel to the chemical only biology believed to date. This finding suggests that so-called safe mobile communications /201100 kHz to 300 GHz/202 across the globe are not safe anymore. The discovery noted above also unravels that from basic protein molecules to the giant biological structure; every part of an integrated biological system absorbs and radiates to communicate in a distinct frequency band. This means, multiple frequency bands operate simultaneously in a biological system and if energy is injected at a local part of any band is distributed all over the entire system. Thus, the biological operational mechanisms and its evolutions are not discrete events as we are taught. Even a linguistic conversation using sound can affect a molecular scale phenomenon and rhythms are eventual expressions that emerge from those molecules. Thus, biological systems like our brain, organs change in every living life form at every moment, define and redefine rhythms of our life /201e.g. circadian, cardiac/202 according to our environment. Our objective is to develop the algorithm that correlates the environment, biological rhythms and the molecular dynamics at the atomic scale, the triangular connecting route will create a new biology, observed but never seen before.

    The global positioning system (GPS) market is a fast changing, highly competitive market. Products change frequently as they try to provide the best customer experience for a service that is based on the need for real-time data. Two major functions of the GPS unit are to correctly report traffic jams on a drivers route and provide an accurate and timely estimated time of arrival (ETA) for the driver whether he/she is in a traffic jam or just following driving directions from a GPS unit. This study measures the accuracy of traffic jam reporting by having Personal Navigational Devices (PNDs) from TomTom and Garmin and phone apps from TomTom, INRIX, and Google in the same vehicle programmed to arrive at the same destination. We found significant differences between the units in terms of their ability to recognize an upcoming traffic jam. We also found differences in how well the devices responded to jams when driving on surface streets versus highways, and whether the jams were shorter or longer in length. We see potential for auto manufacturers to employ real-time traffic in their new vehicles, providing potential growth for real-time traffic providers through access to new vehicles as well as the aftermarket.

    Researchers in the area of cognitive radio often investigate the utility of dynamic spectrum access as a means to make more efficient use of the radio frequency spectrum. Many studies have been conducted to nd ways in which a secondary user can occupy spectrum licensed to a primary user in a manner which does not disrupt the primary user's performance. This research investigates the use of opportunistic access in a frequency hopping radio to mitigate the interference caused by other transmitters in a contentious environment such as the unlicensed 2.4 GHz region. Additionally, this work demonstrates how dynamic spectrum access techniques can be used not only to prevent interfering with other users but also improve the robustness of a communication system.

    Since the most common source of stimulation that initiates AD is the genitourinary tract including bladder distention, followed by colorectal distension, elimination of activation of bladder sensory pathways during bladder distention could significantly reduce the incidence and/or degree of AD in SCI. Because previous studies have indicated that increased levels of nerve growth factor (NGF) in sensory pathways are one of the key factors to induce increased excitability of sensory pathways after SCI, anti-NGF therapy could be an attractive treatment of AD in SCI patients. However, systemic anti-NGF treatment such as the use of NGF antibodies reportedly induces some side effects. Therefore, we hypothesize that the local therapy of NGF antisense delivery using liposomes (LPs) in the bladder could reduce the activation of bladder sensory pathways, thereby suppressing AD during bladder distention after SCI. Using adult female rats with chronic spinal cord injury induced by Th4 spinal cord transection, we will investigate: (1) the contribution of hyperexcitable bladder sensory pathways in the emergence of AD in SCI (Aim 1), and (2) the effects of intravesical delivery of NGF antisense-liposome conjugate, which reduce NGF expression in the bladder, on AD in SCI (Aim 2). If successfully completed, this study directly addresses the feasibility of local NGF antisense treatment for SCI-induced AD and provides the foundation for future clinical translation of local NGF antisense therapy in military service members, their family members, and/or the U.S. veteran population, who suffer from autonomic dysreflexia due to SCI.

    The purpose of this study is to compare a brief-cognitive behavioral therapy (B-CBT) to usual care in the treatment of active duty Service Members who report suicidal ideation with intent to die or those who make a suicide attempt. Enrollment, assessment, and treatment of study participants continues as planned without problems. To date, 161 subjects have consented to participate, of whom 140 have been randomized. 3-, 6-, 12-, and 18-month assessments have been initiated. Recruitment is occurring at a fast pace, with target enrollment (i.e., 150 participants total) projected to be achieved within the next month.

    This work aimed to test the hypothesis that MR guided pulsed HIFU exposures enhance gene delivery and increase the efficacy of gene therapy in inhibiting prostate cancer growth in vivo, particularly when combined with AD or RT. We developed techniques for the treatment of prostate tumor-bearing mice using a clinical MRgHIFU device. We performed animal studies for quantitative measurement of the doxorubicin concentration in HIFU treated prostate tumors to evaluate the optimal ultrasonic parameters derived from experiments using an acoustic phantom. We also performed experiments on the efficacy of MRgHIFU enhancement of docetaxel delivery combined with RT in inhibiting prostate tumor growth in vivo. We investigated the MRgHIFU effect on the enhancement of gene therapy using AS-MDM2 and bcl-2 in implanted prostate tumors in mice in vivo by measuring the protein expression level of MDM2, p53 and p21 using immunohistochemical staining and west blotting techniques. We also performed experiments on the efficacy of MRgHIFU enhancement of AS-MDM2 delivery in inhibiting prostate tumor growth in vivo. Our results show that MRgHIFU is safe and effective for the enhancement of drug delivery in prostate tumor.

    The VA Office of Inspector General Office of Healthcare Inspections conducted an inspection to determine the validity of allegations regarding quality of care at the Marion VA Medical Center, Marion, IL. Four complaints were received between October 2011 and January 2012 regarding the clinical practice of two physicians. In February 2012, Senator Richard J. Durbin forwarded additional allegations concerning one of the physicians. This report addresses allegations regarding quality of care provided for the three patients named by complainants, and describes findings from our site visit. Not addressed in this report are allegations regarding quality of care for additional patients not specifically identified, and allegations regarding a second physician. In the care of one patient, the risk of complications requiring urgent intervention should have been discussed with the patient as part of the informed consent process prior to a procedure, at which time the patients therapeutic preferences could have been clarified. We identified no deficiencies in quality of care for the other two patients. We also found that a physician who was hired after not being in clinical practice for many years was granted clinical privileges with the understanding that his competence would be confirmed by direct observation. However, competence was never documented for invasive procedures that he subsequently performed.

    Over the course of the last year, the study team received IRB approval and began recruiting participants at five of the six study sites (Joint Base Lewis-McChord, Ft. Bliss, Ft. Campbell, Ft. Carson, and Ft. Stewart). As of August 31, 2012, 368 total referrals across the five active sites had been received; 179 participants had been enrolled and randomized into the study (86 participants into the STEPS UP arm; 93 participants into the optimized usual care arm); 53 participants completed the 3-month follow-up assessment; and 6 participants completed the 6-month follow-up. We are awaiting HRPO approval of our final site (Ft. Bragg); we anticipate beginning recruitment at Ft. Bragg in September 2012. Multiple amendments have been approved by the WRNMMC IRB, including an amendment that revised eligibility criteria and updated data collection forms. The study intervention has been refined to include a web-based care management support tool; nurse-assisted web- based cognitive-behavioral therapy options for PTSD and depression; a modularized telephonic cognitive-behavioral therapy option for PTSD and depression; and a preference-based stepped care approach to primary care PTSD and depression treatment sequencing. We have continued to hire and train site and centralized personnel.

    The Wilms' tumor gene, WT1, encodes transcription factors that regulate cell proliferation, differentiation, and apoptosis. WT1 protein is highly expressed in malignant pleural mesothelioma (MPM), and is a rational target for immunotherapy. We have developed a vaccine comprised of four WT1 heteroclitic peptides that are given together with Montanide and GM-CSF as immunologic adjuvants. This WT1 vaccine was previously tested in a small pilot trial, and shown to be safe and immunogenic. We have chosen to test the efficacy of this vaccine in MPM patients who have minimal disease burden after completion of multimodality therapy, but remain at exceedingly high risk for recurrence. The specific aim of this project is to conduct a multicenter, blinded, randomized trial comparing treatment with the WT-1 peptide vaccine + Montanide/GM-CSF to treatment with Montanide/GM-CSF alone in patients with MPM who have completed multimodality therapy. The primary endpoint is progression free survival. The trial has opened at Memorial Sloan-Kettering and is actively enrolling patients.

  • 2110.加速个性化治疗结肠癌的协作模型

    Colorectal cancer (CRC) represents a major health burden, and is the third leading cause of cancer deaths in the U.S. In the past decade, the median survival among patients with metastatic CRC has significantly improved, primarily due the development of active chemotherapeutic regimens that include biological agents. However, despite this success, patients soon run out of therapeutic options and receive salvage therapy that results in only a few weeks of disease stability. We have proposed to employ a team science, systems biology based approach to rapidly identify novel anti-cancer agents and individualize therapeutic strategies in preclinical CRC models. In this Year 1 Progress report, we will present the tasks and key accomplishments achieved within this period of time. In brief, we have completed in vitro testing on a large panel of CRC cell lines for six novel anti-cancer agents. We have completed baseline gene expression profiling of our CRC cell lines panel and patient-derived CRC tumor explant models by high-throughput RNA sequencing approach. We have initiated the in vivo cell line derived xenograft models to test the efficacy of these novel anti-cancer agents and in the process of determining the down stream effectors of these targets by immunoblotting assays. Our research findings for RNA-seq analaysis will be presented at the 24th EORTC-NCI-AACR Symposium on Molecular Targets and Cancer Therapeutics, Dublin, Ireland (November 6-9, 2012). In summary, we have accomplished all the tasks that we proposed in year 1.
