目的：了解我国大型公立医院院长对公立医院改革主要措施的关注情况及态度。方法：随机抽取200名三级公立医院院长进行横断面调查。结果：公立医院院长最关注的问题是增加政府投入（83.7%），对推行临床路径关注比例最低（19.7%）。院长认为政府平均投入应占医院总收入的50.8%，且55.1%的院长认为政府应该按项目对公立医院进行投入。82.0%的院长认为收入未提高是影响医务人员参与医院改革积极性的主要因素，并且仅14.5%的院长对自己目前的收入水平是满意的。结论：三级公立医院院长最关注医院运营相关的改革措施，其次是薪酬制度和院长责权改革。因此，公立医院改革应建立科学合理的医疗服务价格体系以及长效的政府投入机制。建立符合医疗行业特点的岗位绩效薪酬制度，调动医务人员积极性。明确院长责权，落实公立医院经营管理自主权。

目的：评估福建省医保药品支付标准改革对定点医疗机构临床用药的影响。方法：分析福建省省本级、福州市医疗保险管理中心331万参保人员2016年4～9月及上年同期门诊和住院药品消费数据，按年度统计药品费用、用药情况以及医保药品支付标准政策执行情况。结果：改革前后医保药品支付标准目录内费用占比从22.50%上升为64.95%；公立医院及非公立医院改革前后门诊药品前10位一致仅排序略有变化，改革后两者门诊支付标准内费用占门诊费用比例分别为73.21%、39.30%；公立医院住院前10位药品品种及其金额变化较大，其中联合采购价差较大的药品费用增幅较高，而非公立医院相对稳定。结论：医保药品支付标准改革显著影响了临床用药品牌选择和用药结构；以药品集中采购结合联合带量采购形成的医保药品支付标准有利于降低药品价格；医保药品支付标准改革结合医药卫生体制综合改革措施更有利于发挥引导作用。

随着新医改、健康中国2030规划以及“十三五”健康扶贫工程的推进，贫困县持续获益，但农村卫生系统可持续发展终究要变“输血”为“造血”，这需要从卫生规划到治理常态机制的结构性变革。随着中央对农村卫生投资重点的抓大放小、增量发展的抑公促私，贫困县“十三五”卫生规划制定过程充满了博弈、抉择和变通。对西部4个国家级贫困县访谈调研发现，贫困县卫生规划偏好“上项目—拉投资”，呈现出资源规划的增量投机和保守治理等特点。贫困县卫生规划除了要平衡公共利益和部门利益之外，纵向问责控制结构、引民资促发展思路也影响着贫困县卫生规划与治理。
